Description

RESPONSIBILITIES/DUTIES:
Facilitate the day to day operations of the Group Home
Develop, implement, and update Personal Centered Plans (PCP) for each resident which includes but not limited to personal care, developmental interventions, behavioral supports, medical needs, social and recreational (Transportation) care and activities
Develop SMART goals with Residents and their circle of care annually and teach these to QLAs
Review progress of resident goals quarterly and develop strategies for success
Assist Team Lead if needed with the development and implementation of Day Programing
Plan, develop and implement recreation activities
Act as the single point of contact with each residents school/programing/work concerning individual issues, needs and desires
Ordering of all Supplies (pharmacy, grocery, and meal planning, etc.)
Be the main point of contact with the Facilities Department
Maintains and administers monthly budgets (Home and Personal), banking and works with trustees and residents
Provide a nurturing atmosphere for all residents
Lead, monitor and implement systems for residents and QLAs
Provide mentorship to QLAs
Oversee hiring of new supports with manager where appropriate
Provide thorough orientation to new employees and oversee hands on orientation
Plan and facilitate monthly staff meetings and provide minutes to manager and staff
Oncall duties as per rotation
Assume any responsibilities as directed by the manager and provide leadership in managers absence

 - Supervise activities of clients living in group homes and half-way houses
 - Implement behaviour management programs
 - Provide personal services support to persons with special needs
 - Establish registry of special needs support workers
 - Liaise with other social services agencies and health care providers involved with clients
 - Develop service intervention logic models and outcome measures
 - Assist in evaluating the effectiveness of treatment programs
 - Screen, recruit and train volunteers and support staff
