Elmwood operated eleven (11) Community Homes, a 30-space Manor and a Supportive Independent Living Program. We are currently seeking a Community Home Coordinator (CHC) to join our team.

About the Position
Reporting to the Manager of Resident Support, you lead a team of Support Workers to provide a high level of service to residents in our Community Homes. You ensure effective delivery of services that provide needed support to both our residents and the Team Members that support them. You focus your time in three main areas: Resident Support, Home Administration and Team Leadership. This is accomplished while ensuring a safe, respectful, and comfortable environment for our residents and maintaining our Resident First Approach to ensure our residents live fulfilled lives.

Key Responsibilities:
    Lead a team to provide excellent resident support and care.
    Develop and maintain Resident Support Plans.
    Provide ongoing communication to residents families.
    Ensure the home is maintained and has the required groceries and other household supplies.
    Provide on-going skills training and development to Team Members.
    Provide regular coaching and feedback to Team Members.
    Provide support, development and facilitation of Person Centered Plans
    Assist with the support of Residents.

Qualifications
    Completion of Grade 12; related post-secondary is an asset.
    Experience working with individuals living with intellectual disabilities.
    2+ years of supervisory experience.
    Drivers license.
