Job Description: 224 HOURS IN 6 WEEK ROTATION

The Community Social Worker provides client-driven health and social case management assistance to clients to achieve safe and reasonable goals that will optimize clients health and wellness and improve their quality of life. The Community Social Worker works in collaboration with clients, their families and community and government agencies, and works closely with the Population Health Unit and local primary care and public health teams.
The main goals are to assess, plan, implement, coordinate, monitor and evaluate the services required to meet the health and human service needs
of clients living with chronic conditions such as HIV and Hepatitis C, who have difficulty engaging in care. The Community Social Worker also acts as a resource to and supports the development of community and interagency initiatives.

Qualifications and Experience:
- Bachelors degree in Social Work.
- Membership with Saskatchewan Association of Social Workers (SASW)
- Possession of a valid drivers licence.
- Experience in case management in a cross-cultural setting considered an asset.
Skills and Abilities:
- A wide range of supportive counselling skills, including crisis intervention skills
- Knowledge of northern health system and community resources and the skills to make appropriate assessments and referrals within a complex
system of services
- Knowledge of the impact of social determinants of health on individuals living at risk in the community who have diverse challenges and needs
- Ability to apply a philosophy of harm reduction and strength based, client centred care to all interactions with clients, their families and their support
teams, working with clients and families where they are at, building trusting relationships
