Job Description: Reporting to the Clinical Integration Manager, the Palliative Care Coordinator is responsible for providing day-to-day direction and monitoring of palliative care services provided in a specific geographical area within the Sunrise Health Region. In so doing, the Palliative Care Coordinator ensures that the care provided is in accordance with the Sunrise Health Region Mission, Palliative Care Philosophy, client care plan, district policies, procedures and standards of care.
Required Qualifications

Bachelor Degree/Diploma in Nursing
CNA Certification in clinical practice area
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Experience
Minimum one (1) year experience in providing palliative care preferred.  
Minimum one (1) year experience in community nursing preferred.
Other Information

Note:  This position is open to all SUN members within the bargaining unit as per Article 27.05 of the SUN Collective Bargaining Agreement, however first preference will be given to employees of Sunrise Health Region.

Successful candidates will be required to provide:
Current Criminal Record Check and Vulnerable Sector Check (less than 6 months old)
Certificate/Diploma/Degree as required for position
Any applicable licenses/registrations relating to the role

Expected Up To Date: August 9, 2022
FTE: 1.00

Hours of Work: Shift Information: Days
224.00 hours per 6 week rotation: 28 shifts of 8.00 hours
