Facility: St Pauls Hospital
Job Description: Under the direction of the Manager of Kidney Health , the social worker assists the healthcare team in the development of Kidney Health. The incumbent assesses and provides counseling to clients and their families dealing with the complexities of kidney disease. The social worker will function as a member of the renal program with an emphasis on the team approach to care. Travel is required for this position. The Social Worker will function as a member of the Kidney Health interdisciplinary team, with an emphasis on patient and family-centered care.
Required Qualifications

Bachelor Degree in Social Work
Registered or eligible for registration with the Saskatchewan Association of Social Workers (SASW)
Experience
Previous experience with medical social work in the field of chronic disease management is required.
Knowledge, Skills and Abilities

Ability to work independently and as a member of a multi-disciplinary team
Advanced project management skills
Ability to assist and develop policies and procedures according to best practices
Ability to develop, evaluate and modify programming provided to clients
Advanced change management skills
Advanced research skills
Assessment skills
Communication skills to provide/obtain verbal and/or written opinions/advice
Supervisory skills

Field Hours
Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement.  HSAS seniority within the Saskatchewan Health Authority will be considered under the Selection Criteria.
