Description

The Aftercare Coordinator will lead development initiatives and manage efforts supporting the SHWC Treatment Centre's mission, vision and value; will ensure the organization will create a culturally safe place for clients. Working with a number of identified organizations that will assist in the vision to providing help, treatment and support for persons, family, community in the area of drug and alcohol addictions and will ensure organizational and program effectiveness occurs through a sustainable and integrated services approach.
Facilitates workshops and training in the area of mental health and addictions
Research options to aid organization in exploring changes that will support the aftercare development of clients.
Evaluation and implementation of existing programs and the start of new programs
Provides oversight/leadership to the team
Implementing program partnerships with respectful cooperative relationships
Evaluation of outcomes to maximise resources, service quality, integration, innovation and integrity
Utilizing and modeling learning principles in leadership and problem solving.
Provide crisis intervention.
Meeting difficult situations, identifying solutions, facilitating the course of action
Enforce program rules/regulations to clients.
Refer clients to other community services available in the region if necessary. E.g. Mental Health and Wellness Services, Child Protection Services, Drug & Alcohol Addictions, etc.
Encourage clients to attend workshops, traditional teachings and ceremonies through services provided in their home communities.
Participate in the acceptance committee of applicants to residential treatment.
Maintain records of services provided.
Aid clients in designing a culturally sensitive personal Aftercare plan.
Abide by the code of ethics of the certification body.
initiate, promote and maintain partnerships with other service agencies in the community. Must be fully vaccinated for Covid-19.
