Position Description

Working in collaboration with the Reaching Home Community Entity and under the overall direction and direct supervision of the Coordinated Access System Manager, this position is an integral part of Reginas Reaching Home Coordinated Access System, and the culturally safe, trauma informed assessment and prioritization for services of homeless individuals and families in our community.  

The CA Intake & Assessment Worker will be responsible to receive referrals from Reaching Home funded homeless serving organizations as well as other community-based entities, will conduct secondary assessments (currently SPDAT), and make referrals for services to the most appropriate agency in accordance with Reginas Coordinated Access system policies and procedures. This position will require community outreach and engagement, client data collection (HIFIS), referral and connection to financial supports, and a variety of duties to assist individuals who are currently experiencing homelessness. The program is very flexible and provides an atmosphere of acceptance and non-judgemental approach when working with individuals.
