The Case Manager/Primary Care Counsellor provides addiction counselling and treatment support to the target population of those who are accessing services from the RAAM (Rapid Access to Addictions Medicine) clinic; and have difficulty engaging or have disengaged from care, and may be active in substance use, or have a primary care provider at BFHC.  This position will work both on First Nation Communities and The Battlefords.

This position is responsible for supporting the providers in BFHC in screening, brief intervention, accessing appropriate services for ongoing mental health and psychosocial services for clients and provide support and coverage for the other members of the RAAM Team as needed.

Qualification and Experience:
* Minimum bachelors degree in Social Work, Psychiatric Nursing, or other comparable discipline with related experience
* Minimum one (1) year experience working with marginalized populations affected by addictions in a health-related field
* Knowledge of the Chronic Disease Management and behaviour change models, addictions, harm reduction theory, teaching/learning principles and motivational interviewing
* Knowledge of HIV, Hep C, and addiction recovery with opioid agonist therapy, knowledge of current trends, issues, and best practices  
* Demonstrated knowledge of community resources and programming supportive of this client population
* Demonstrated performance of appropriate documentation, record keeping and data collection functions
* Demonstrates knowledge and understanding of traditional aboriginal values, beliefs and healing approaches and a willingness to learn about and accept Indigenous cultures
* Self-directed and requires minimal supervision.
* Demonstrate agency values in the work they do daily with BRT6HC First Nations communities, clients, co-workers and partnering agencies.
* Successful criminal record with vulnerable sectors check
