Description

To provide support services that will enhance the well being and social functioning of family units, as well as provide support for those who have been affected by individual or family crisis.

Components of the prevention model include:
*Work with parents/families to assess their needs and develop programs activities that meet those needs
*Manage a caseload of approximately 15 families
*Complete daily contact notes, ensuring, where applicable, copies are made for the prevention program and originals are sent to appropriate family service worker.
*Maintain regular communication and collaboration with protection services, especially the family service worker assigned to the family
*Develop/coordinate/facilitate individual and family enhancement programming/activities in collaboration with Prevention Manager
*Personal coaching services through personal crisis intervention, support and assistance including referral to appropriate professional and community resources.
*Develop/coordinate/facilitate personal and family enhancement programs/activities.

This position will be based in the community of Montreal Lake 106A.
