Description

We are seeking a Community Health Representative (CHR) to join the Standing Buffalo Dakota Team Health Team. The Health Department manages a full spectrum of programs and services supporting community members health and well-being. It is a permanent full-time salaried position.
RESPONSIBILITIES:
Assists in providing health education to the people of Standing Buffalo Dakota Nation, including, but not limited to health promotion and prevention, ensuring awareness of the availability of general health care services;
Coordinates the Chronic Disease Management and Diabetes Programs;
Oversees the Canadian Prenatal Nutrition Program (CPNP);
Support and advocate on behalf of members with external agencies (e.g. NIHB, INAC, Health Canada);
Organizes and arranges for the delivery of health care seminars, workshops, special events and other educational programs and services;
Assists in ensuring the availability of, and access to medical/dental/health programs and services;
Assists health staff in the provision of health programs and services;
Performs home visits as needed in consultation with other health programs;
Prepares work plans, budgets and reports as required;
Performs such other related duties as may reasonably be required by the Director of Health.

KEY REQUIREMENTS:
Graduation from a post secondary program in social services, public/community health, nursing or equivalent is considered an asset.
Excellent working knowledge of public health programs and services.
Extensive experience working in a community health setting including development, delivery, monitoring and evaluation of health, social and senior services programs.
Experience working in a First Nations community setting including knowledge of First Nations culture, traditions and values considered an asset.
Able to supply a clear criminal record check with vulnerable sector.
Valid class 5 Drivers License & Clean Drivers Abstract Required.
