Facility: HOME CARE PRINCE ALBERT
Job Description: Under the general direction of the Manager or designate, provides an accessible, client-centered and integrated approach to the processing of requests for Home Care services; performs initial consultation, preliminary assessment and identification of needs for potential clients; takes and screens referrals from various sources; determines eligibility of clients, initial interventions and the response time needed.
This position is a new position as such requires the successful candidate to identify improvements needed in existing process; and to use creativity and resourcefulness to help improve response time, and ensure appropriate and effective client care in the community. Integrating new and improved intake procedures into the present environment will be a key component of the position.
Required Qualifications
Bachelor Degree/Diploma in Nursing
Basic Life Support (BLS)
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Valid Class 5 driver's license

448 hours of work in a 12 week rotation.
