Description

JOB DESCRIPTION:
-To provide case management function, counselling support, life skills coaching and assessment of client's skills in accordance with individual treatment plans to individuals who are residing in our group home that provides 24/7 residential supports to individuals with a mental illness.
-days are Monday to Friday.  Shifts rotate weekly:  week one is the day shift (8am-4pm) and week two is the evening shift (4pm-12am).
-Primary responsibility is to ensure a safe, supportive and healthy environment for the residents according to our program outline.

REQUIREMENTS:
-- Minimum of two years post-secondary education in social/health sciences
-- Some related work experience would be beneficial
-- Good communication skills
-- Knowledge of mental illnesses

 - Maintain program statistics for purposes of evaluation and research
 - Assist in evaluating the effectiveness of treatment programs
