Job Description: Under the direction of the Manager of Rehabilitation and Residential Services, Mental Health Services, Prince Albert Parkland Health Region, the Senior Assessor Coordinator is accountable for the provision of an appropriate range of community residential options for mental health clients. Using a holistic and client / family centered approach, the incumbent facilitates appropriate assessments, referrals and placements of clients with long term mental illness as well as implementing Mental Health Home Care services for discharged patients.

Under the direction of the Manager of Rehabilitation and Residential Services, and in consultation with Psychiatry, this position has a collaborative responsibility with the Community Mental Health Nursing department and the Patient Care Coordinator Unit, to formulate and manage residential options/discharge plans for long term mentally ill clients.

This position is responsible for organizing and carrying out training sessions for the Mental Health Approved Home operators on an ongoing basis. The incumbent acts as a liaison with Psychiatry.

The work is very comprehensive and diverse in nature, evident by the large number of referrals of chronic mentally ill clients to Rehabilitation and Residential Services on an annual basis.  The position relies on collaboration and cooperation, which is essential when working in this multidisciplinary team. Some travel is required within the region and provincially.

Required Qualifications

Valid Class 5 driver's license
Bachelor Degree/Diploma in Nursing or Bachelor Degree/Diploma in Psychiatric Nursing or Bachelor Degree in Social Work
Registered or eligible for registration with the appropriate licensing body
