Description

The Wound Management LPN will work under the direct supervision of the Home Care Supervisor on matters related to providing objective, non-judgmental assessment of the clients needs, the development of a goal-based plan of care in conjunction and guidance with the Registered Nurse and/or supervisor and will effectively organize Home Care services within the home based on physician orders and instructions in an innovative and adaptable manner to manage the clients needs with consultation of the Registered Nurse and/or supervisor; which will also include providing personal care to clients whose condition cannot be safely carried out by the Home Health Aide or family members.

QUALIFICATIONS
* Registered Nurse in good standing (or eligible to become registered) with Sask. Registered Nurses Association.
* Licensed with the Saskatchewan Association of Licensed Practical Nurses
* Minimum 3 years in community health nursing experience in First Nations community an asset.
* Demonstrate an understanding of the health and social issues facing First Nations people
* Good understanding of population health principles and concepts
* Must be able to work as an effective team member and to work independently, in a self- motivated manner.
* Excellent interpersonal communication skills to communicate openly and effectively with patients and families.
* Willingness to travel and possession of a valid Class 5 drivers license, work flexible hours, possibility of some evenings and some weekends
* Previous working experience with First Nations people and demonstrate an understanding of the health and social issues encountered by First Nation people and a willingness to learn about and accept First Nations cultures.   Knowledge of Plains Cree Language is a definite asset.
* Must provide a clear criminal record check with vulnerable sector
* Must be computer literate to accurately document clients EMR record.
