Job Description: PROVIDE ASSESSMENT OF CLIENT NEEDS AND CLINICAL COORDINATION FOR A DEFINED GROUP OF PATIENTS IN THE NEPHROLOGY PROGRAM REGARDING MEDICAL STATUS, CLINICAL PROBLEM SOLVING, PATIENT/STAFF EDUCATION AND PARTICIPATION IN RESEARCH. MONITOR AND TRACK PATIENTS PLACED ON THE WAITING LIST TO ASSESS FOR CHANGES IN CLINICAL CONDITION AND PRIORITIZE AS NECESSARY. ASSESS PATIENT NEEDS/CONCERNS THROUGH CHART REVIEW, HISTORY TAKING, LAB DATA AND IN CONSULTATION WITH ATTENDING PHYSICIAN. COMMUNICATE WITH PHYSICIANS AND NURSING UNITS TO MANAGE PATIENT FLOW. ASSESS LABORATORY RESULTS AND CONSULT WITH PHYSICIAN. PROVIDE EDUCATION TO PATIENTS AND FAMILY IN PREPARATION FOR, AND FOLLOW UP OF, NEPHROLOGY TREATMENT AND PROCEDURES. PROVIDE OTHER EDUCATIONAL NEEDS FOR NURSING UNITS, PHYSICIANS AND OTHER STAFF AS NECESSARY. IDENTIFIES, ORDERS AND PRIORITIZES DIAGNOSTIC TESTS AND PROCEDURES IN CONSULTATION WITH THE PHYSICIAN. LIAISE WITH STAFF TO COMMUNICATE INFORMATION AND FACILITATE FOLLOW UP CARE. PARTICIPATE IN DEVELOPMENT, MONITORING AND EVALUATION OF PROTOCOLS, GUIDELINES AND STANDARDS OF CARE IN COLLABORATION WITH PHYSICIAN AND NURSING STAFF.
Required Qualifications
Bachelor Degree/Diploma in Nursing
Basic Life Support (BLS)
CNA Certification in clinical practice area
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Experience
5 years recent full-time experience in Nephrology required.
