We are currently looking for Community Care Partners to join our team in Moose Jaw where we operate group homes providing holistic support to adults with intellectual and physical disabilities. RESPONSIBILITIES:
DIRECT SUPPORT
Be a part of a support circle that helps residents and their families to identify daily care needs.
Be a part of an interdisciplinary team that provides the necessary supports for individuals to thrive in their daily lives.  
Practice observation skills as you monitor resident's progress toward their personal goals.
Assist people to take their medications as needed.
Assist residents to fully engage with the wider community through recreational opportunities, community events and accessing community resources.
INDIRECT SUPPORT TASKS
Keep accurate records about important events and processes in the residents' lives.
Assist the residents with managing their home.
Work with equipment needed for support which include mechanical lifts, agency vehicles and household appliances.
MINIMUM QUALIFICATIONS:
Grade 12
Full Class 5 Driver's Licence (not a Novice 1 or Novice 2) and submission of Driving Abstract
Satisfactory Criminal Record Check with the Vulnerable Sector Check
