Description

Provide support to persons experiencing mental health and addictions struggles with culturally competent and culturally safe support services until such time as they can transition into Saskatchewan Health Authority longer addiction care facilities, supports and services, this filling a systems gap between the time a person identifies needing support and receiving it.
Providing case management, peer navigation and long-term case planning with persons as they move from immediate to long-term care facilities supporting treatment and recovery.
Work across systems to ensure Indigenous communities are supported with HIV prevention including culturally competent rapid testing for both HIV and COVID-19 where available, peer navigation/mentor supports, peer advocacy/housing services, food security and basic needs services.
Provide education and awareness of evidence-informed and best practice for addictions support, HIV and COVID-19 vaccine uptake to Indigenous harm reduction practices using person-centered, strength-based frameworks.
Ensure persons are not released to homelessness upon discharge from care facilities by providing access to housing services case management.
