Facility: REGIONAL HOME CARE
Job Description: The Palliative Care Coordinator is responsible for the development, implementation and evaluation of home and facility based palliative care services.
Required Qualifications

Bachelor Degree/Diploma in Nursing
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
LEAP certification (Learning Essential Approaches to Palliative and End of Life)
Experience
Minimum of five years of palliative/community care work experience preferred. Certificate of completion from a Palliative Care Training Program is desirable. Demonstrated interest, skill and experience in working with terminally ill clients and their caregivers. Proven ability to work as a member of the Home Care and palliative care teams.
Knowledge, Skills and Abilities

Organizational skills
Ability to work independently and as a member of a multi-disciplinary team
Decision making skills
Ability to communicate with and maintain an effective working relationship with clients from diverse social, economic and cultural backgrounds
Ability to make decisions, which must include coordination of staffing, and/or patient care, and/or coordination of equipment
Ability to travel both within the region and provincially
Excellent verbal and written communication skills
Ability to conduct assessments, prioritize client needs, and develop treatment plans to meet needs regarding treatment with a client and family-centered approach to care
