Job Description: The Primary Health Care (PHC) RN is an integral member of the primary health care team, providing and coordinating initial, continuing and comprehensive nursing services to patients in the PHC sites.
The PHC RN promotes quality health care outcomes working within established standards for case management practice.
The PHC RN initiates and/or participates in community development initiatives and participates in the evaluation of PHC initiatives within the site.
The PHC RN takes a lead in developing, implementing and evaluating health promotion and screening programs within the clinic.
The PHC RN collaborates proactively with interdisciplinary team members and with a patient focus to facilitate and maximize optimal health care outcomes.

Required Qualifications

Bachelor Degree in Nursing
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Valid Class 5 driver's license
Knowledge, Skills and Abilities

Interpersonal skills
Ability to work independently and as a member of a multi-disciplinary team
Communication skills
Ability to adapt to changing conditions
Ability to competently use computers and software applications as required by the position
Assessment skills

2 years nursing experience in a community based health care setting preferred.
Knowledge of chronic disease management.
Experience in case management an asset.
Level C, Basic Cardiac Life Support.

This is a designated field hours position.

Geographic Location: Lloydminster
Expected Start Date: August 29, 2022
Expected Up To Date: February 28, 2023
FTE: 1.00

Hours of Work: In a 6 week rotation: 28 shifts of 8.00 hours
