Job Description: The Case Manager/Assessor is responsible for providing assessment, care planning, client advocacy, and coordination or services and monitoring of care needs. As a team member, the Case Manager will participate in policy and program development, health education, and promotion and liaison with other health professionals/service sectors in the community.
Required Qualifications

Basic Life Support (BLS)
Bachelor Degree/Diploma in Nursing or Bachelor Degree/Diploma in Psychiatric Nursing or Bachelor Degree in Social Work
Valid Drivers License
Ability to conduct a mental health assessment of a client including obtaining collateral information from caregivers and families.
Current licensure with applicable provincial licensing body
Experience
Previous clinical experience and background in Home Care or HCCM would be an asset.
Knowledge, Skills and Abilities

Ability to communicate with and maintain an effective working relationship with clients from diverse social, economic and cultural backgrounds
Ability and willingness to travel and work flexible hours including evenings/weekend as required
Ability to screen for addictions and/or addiction related issues and make appropriate referrals
Ability to handle multiple tasks and deadlines
Ability to follow safe work practices
A demonstrated ability to meet and work with clients, utilizing sound judgment, tact, initiative and high standards of integrity and responsibility
Ability to administer and interpret various scales to determine a client's mental status.
