Description

Reporting to the Intensive Case Manager, Housing Programs the Community Integration Support Worker is responsible for the provisions of housing stabilization and intensive case management services for those that are homeless or at risk of being homeless. This position will provide intensive case management and supportive services to the highest acuity clients that are referred from the CAR program.
Job Duties Include:
- Receiving referrals from the CAR program.
- Screen and assess at risk clients.
- Safety planning and implementation of case pans and case management reporting.
- Secure housing for high acuity clients.
- Provide in-home life skills mentoring.
- Make referrals to internal and external programs.
- Maintain and report on program statistics and outcomes.
- This position will require some on call work as the program supports 12 high needs individuals with 24/7 support.
The ideal candidate will have:
- Bachelors of Indigenous Social Work/Social Work or Humanities, however, equivalencies based on education and experience may be considered.
- A valid driver's license is required.
- Demonstrated experience and understanding working with marginalized people and people of diverse cultures and abilities in a community-based setting.
- Demonstrated experience working with high needs clients that present with complex mental health and addition issues.
- Knowledge of Indigenous culture and the impacts of intergenerational trauma.
- Ability to work in fast paced and high stress environment.
- Excellent judgement, discretion, diplomacy, strength-based, solutions-focused team player.
- Excellent communication skills, both written and oral.
- Knowledge of supportive services available in Regina.
