Job Purpose:
Provides, information service for advocacy, systems navigation for people with disabilities
Service Delivery Responsibilities:
Assess cases with tools provided for the purpose of evaluating the need for service and/or referral for service
Identify applicable services and community resources for clients and liaise with appropriate agencies
Advocates with client to provide a fair and equitable environment to address issues of disagreement with governments departments and/or agencies. Demonstrates conflict resolution skills
Fields inquiries
Educates individuals on rights and responsibilities of tenancy
Participates with networks to address disability needs in the community
Gathers data, updates and maintains records regularly for statistical purposes
Assist individuals with filling out disability-related forms
Helping people prepare for government program appeals
Providing information about disability services and programs
Skills/Qualifications
Person centered service delivery, which includes: program knowledge, quality focus, problem solving, documentation skills, listening, phone skills, resolving conflict and analyzing Information
Working knowledge of the social and economic barriers of the vulnerable people with disabilities in the community
Working knowledge of community resources including outreach, programming and services
