Job Summary
With direction from Willow Cree Health Services Corp (WCHS), the incumbent will support and adhere to the Mission & Vision of the organization to provide Home Care services to the community members of Beardys & Okemasis Cree Nation (BOCN). The HCC Coordinator will be a liaison between outside agencies and clients.  The HCC Coordinator will be required to complete administrative duties such as work plans, budgets, reports and evaluations. The HCC Coordinator should possess excellent interpersonal skills and be a team player.  Strict confidentiality of sensitive matters is a requirement.  The HCC Coordinator reports to the Director of Health.

Job Responsibilities
    Adhere to the Willow Cree Health Services Personnel Policy & Procedures Manual.
    Maintain strict confidentiality.
    Supervise the Home & Community Care Department.
    The HCC Coordinator must also maintain statistical and prepare monthly reports and all reports required by community and funding agencies.
    Assist identifying the need for family support services and available resources.
    Coordinate case plan management, identifying short and long term goals for clients.
    Maintain confidential, current, accurate and complete client documentation.
    Develop an annual work plan, annual budget, reports and staff evaluations.
    Participate in training, education, mentoring and other forms of professional development.
