Elmwood Residences Inc. is a non-profit organization committed to supporting those living with intellectual disabilities. We believe in providing choices and opportunities to support learning and growth, fostering understanding, respect and advocating for all individuals.

As a Health Connector you will support our residents through ongoing healthcare assessments and care plans based on each individuals needs. The Health Connector supports the Manager of Health & Wellness in employee education and acts as a liaison with healthcare services.

Responsibilities:
-Assist in medication and insulin administration training for employees
-Develop, audit and maintain health passports, care plans, and medications related to resident specific health and medical information
-Communicate with other healthcare services as needed
-Provide general healthcare related tasks including, but not limited to;
Wound care, Skin Assessments, Catheter Care, UTI Checks, Administer injectable medications, Assist in home and health related assessments, and assist in resident appointments.

Qualifications:
-Current Nursing Registration in the Province of Saskatchewan
-Community College graduate with a RN, LPN or RPN Certification
-Minimum of 2 year nursing experience
-Strong knowledge of intellectual disabilities, application of intellectual disabilities to the aging process, psycho-social and family issues
-Able to set priorities and meet deadlines in a dynamic and changing environment
-Demonstrate strong communication skills
-Demonstrate a passionate commitment to people with intellectual disabilities and improving the resident's quality of life
- Able to work with a multi-disciplinary team through collaboration, joint decision making and building integrated action plans
-Experience in the intellectual disability field is an asset
