THE POSITION
The Wicihitowin Nurse Practitioner, under the direction of the STC Health Centre Manager, will collaborate with members of the interdisciplinary team to plan and implement the Wicihitowin Healing in Community program. Working in an outreach capacity, the Wicihitowin Nurse Practitioner will be primarily responsible for creating an individualized plan of care, monitoring and evaluating an individuals progress through the program, coordinating care, health promotion, and ongoing support for individuals and their families.
