Job Description: REPORTING TO THE MANAGER OF CHRONIC KIDNEY DISEASE - RENAL SERVICES AND HOME BASED THERAPIES, THE PATIENT EDUCATOR RESPONSIBILITIES WILL INCLUDE: COORDINATE AND PARTICIPATE IN PATIENT EDUCATOR FOR CHRONIC KIDNEY DISEASE PATIENTS REQUIRING PERITONEAL DIALYSIS AND HOME HEMODIALYSIS. THIS WILL INCLUDE LIAISON WITH FAMILIES, PHYSICIANS, DIETITIANS, STAFF NURSES, AND COMMUNITY AGENCIES. PROVIDE INDIVIDUAL OUTPATIENT EDUCATION FOR PATIENTS AND FAMILIES IN A CLINIC SETTING. COORDINATE AND PARTICIPATE IN EDUCATION PROGRAMS FOR DESIGNATED NURSING STAFF THAT WILL BE PROVIDING PERITONEAL DIALYSIS FOR PATIENTS AT SPH AND OTHER AGENCIES. SERVE AS A PRIMARY CLINICAL RESOURCE FOR NURSING, MEDICAL AND OTHER PROFESSIONAL/SUPPORT STAFF. PARTICIPATE IN DEPARTMENTAL ACTIVITIES (PROGRAM DEVELOPMENT, PLANNING, QUALITY IMPROVEMENT, AND EVALUATION). PARTICIPATE IN RESEARCH AND EVALUATION PROJECTS.
Required Qualifications
Bachelor Degree in Nursing
Basic Life Support (BLS)
Registered or eligible for registration with the College of Registered Nurses of Saskatchewan (CRNS)
Experience
3 yrs related clinical nursing experience with patients and families with chronic disease, in particular chronic kidney disease is required.
Knowledge, Skills and Abilities
Ability to competently use computers and software applications as required by the position
Ability to travel both within the region and provincially
Ability to work independently and as a member of a multi-disciplinary team
Advanced assessment skills
Communication skills to transfer knowledge/skills
