Facility: THERAPIES
Job Description: The Coordinator, Acquired Brain Injury Program, report to the Manager of Integrated Therapies & Stroke Services, and is responsible for delivery, coordination and implementation of community resources and services on behalf of the client with a brain injury within the Sunrise Health Region.  This will include case management services and education to individuals with an acquired brain injury, family members, caregivers and other service providers throughout the Sunrise Health Region.  the Coordinator will provide care as outlined in the proposal document entitled Putting Our Heads Together in East Central Saskatchewan"". The Coordinator will assist with and contribute to program development.
Required Qualifications

Bachelor Degree/Diploma in Nursing or Bachelor Degree in Social Work or Bachelor Degree in Physiotherapy or Bachelor Degree in Occupational Therapy
Bachelor Degree/Diploma in Nursing or Bachelor Degree/Diploma in Psychiatric Nursing or Bachelor Degree in Social Work
Masters Degree in Speech Language Pathology
Experience
Experience in case management and counseling is an asset.

Knowledge, Skills and Abilities

Ability to communicate with and maintain an effective working relationship with clients from diverse social, economic and cultural backgrounds
Ability to motivate clients
Ability to travel both within the region and provincially
Ability to work collaboratively in a team environment
Advanced assessment skills
Advanced interpersonal skills
Basic computer skills
