In collaboration with the Allied Health Professionals (Occupational Therapist, Physical Therapist and Speech and Language Pathologist), the Physical Wellness Worker coordinates their community visits, performs related tasks assigned following the care plan, and documents and reports the client's progress. Providing home visits to deliver physical therapy services is part of the role of the incumbent.
In collaboration with other departments and social services agencies, including the school, the incumbent will lead the development and implementation of programming promoting physical activity targeting children 0-18.
