Job Description: Outreach workers will apply the principles of health promotion, population health, and harm reduction to positively impact individuals, families, groups and communities. In addition to providing client care coordination and support, outreach workers will aim to improve the social determinants of health in the region through advocacy and partnerships.

The outreach worker collaborates within a multidisciplinary team to establish, monitor and carry out specific care plan activities to link, engage and retain clients to care using a harm reduction model.  This involves but is not limited to searching out clients who are lost to care; checking in with clients to assess progress towards care plan activities; referring clients to social and health agencies; intervening in crisis situations to ensure client safety; arranging and accompanying clients to appointments related to health or social issues; distributing and exchanging harm reduction supplies in a variety of settings; providing basic health education on blood borne pathogens, needle safety and disease prevention; contributing to the content of related health promotional materials; connecting with clients and community to promote program services in a variety of public settings; advocating for improved health and social services to address barriers to daily living and other related duties as assigned.

Required Qualifications

Bachelor Degree in Social Work
Registered or eligible for registration with the Saskatchewan Association of Social Workers (SASW)
Valid Class 5 driver's license
Knowledge, Skills and Abilities

Ability to adapt to changing conditions
Ability to competently use computers and software applications as required by the position
