Description

Under the direction of the SIMFC Transitional Housing Coordinator, the SIMFC Transitional Housing Support Worker will work closely with the SIMFC Housing staff in providing a safe home for clients who are in the process of transitioning from homelessness to living in a supportive home where the end goal is for the clients to gain the skills and knowledge to transition to living in their own place.

Duties and Responsibilities

The SIMFC Transitional Housing Support Worker will:
Work with clients who reside in our Transitional Home to provide support and guidance with the intention of the client moving to a sustainable housing situation that reduces their vulnerability to homelessness
Case plan with clients and assist them in working toward becoming self-sufficient and responsible individuals.
Organize and co-ordinate self-help workshops.
Daily log notes
Act as liaison and advocate with Landlords, Social Services and other agencies.
Maintain up-to-date list of referral agencies and make periodic contact with these agencies to ensure a good working relationship.
Record statistical information on each client.
Engage in the fundraising and community engagement activities hosted by the Friendship Centre
Be responsible for data and stats collection
Report writing when required
Other duties as assigned
