Job Description: UNDER THE DIRECTION OF THE MANAGER OF THE RENAL PROGRAM, THE SOCIAL WORKER ASSISTS THE HEALTH CARE TEAM IN THE DEVELOPMENT OF THE RENAL PROGRAM. THE INCUMBENT ASSESSES AND PROVIDES COUNSELING TO CLIENTS AND THEIR FAMILIES DEALING WITH THE COMPLEXITIES OF KIDNEY DISEASE. THE SOCIAL WORKER WILL FUNCTION AS A MEMBER OF THE RENAL PROGRAM WITH AN EMPHASIS ON THE TEAM APPROACH TO CARE.
Required Qualifications

Bachelor Degree in Social Work
Registered or eligible for registration with the Saskatchewan Association of Social Workers (SASW)
Experience
PREVIOUS EXPERIENCE WITH MEDICAL SOCIAL WORK IN THE FIELD OF CHRONIC DISEASE MANAGEMENT IS REQUIRED..

Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement.  HSAS seniority within the Saskatchewan Health Authority will be considered under the Selection Criteria.

Hours of Work: Shift Information: Days
In a 6 week rotation: 21 shifts of 8.00 hours
