Facility: LA RONGE HEALTH CENTRE
Job Description: Reporting to the Manager of Mental Health and Addiction Services, the incumbent is responsible for the screening and coordination of initial service requests for all programs within Mental Health and Addictions Services in the former health region. The Intake Facilitator provides screening, triage, assessments for urgency/risk, and coordinates case assignments. The position facilitates crisis response and short-term support for services to clients presenting with a variety of mental health and addictions issues. The Intake Facilitator provides consultation to Community Agencies and Programs regarding services available in Mental Health and Addiction Services.  

Qualifications:
-One of the following: Bachelor of Social Work, Bachelor of Arts (Social Sciences), Bachelor of Science in Nursing, Bachelor or Diploma of Psychiatric Nursing or a Addiction Counseling Degree
-Current registration in corresponding professional association.
-Previous experience in the Mental Health and Addiction Services.
-A drivers license and ability to travel.

Knowledge, Skills and Abilities

Ability to conduct a suicide risk assessment and create a risk management plan with the other health care providers on the team.
Ability to work independently and as a member of a multi-disciplinary team
Apply ethical, culturally sensitive and legal principles in practice with peer supporters and clinicians working with at-risk individuals and their families. This includes sound Knowledge of protocols and Legislation that relate to addictions and mental health services and the practice of peer support.
Awareness of and ability to cope with a wide variety of clinical, ethical and legal issues related to mental health services.
