Description

The Parent Mentor reports directly to the Health Director of Muscowpetung Saulteaux Nation. The Parent Mentor will assist families to develop parenting skills which will support them in raising healthy babies and young children in an environment of safety, growth and learning. The Parent Mentor will work closely with the Community Health Nurse(s) delivering the Maternal Child Health Program, utilizing community-based resources as appropriate.

DUTIES AND RESPONSIBILITIES:

-Together with the Maternal Child Health Coordinator and Community Health Nurse/Community Team;
-Promote the Maternal Child Health Program
-Manage a caseload of clients and their families
-Conduct home visits as per individualized plan of care
Make and keep appointments for family visits in the home or other meeting place
-Use communication skills to develop a working relationship with the women and create a healthy support network
-Emphasize traditional and cultural aspects
-Offer support, encouragement and reassurance to women and their families
-Refer at risk women to the CHN and/or Maternal Child Health Program Coordinator
-Participate in training programs
-Complete required forms, charting and reporting requirements
-Maintain client confidentiality and be professional at all times
-Help develop parenting skills for women and their families
-Participate in regular team meetings to evaluate client and program process
-Commit to long term involvement and flexible services
