Job Description: Reporting to the Director, PHC or his/her designate, Primary Health Care Teams and in collaboration with physicians and health care provider team members, the Registered Nurse (Nurse Practitioner) will provide Primary Health Care (PHC) services to assigned populations which may include: urban populations, visiting services to rural communities, and/or priority populations (e.g. newcomers, teens, residents in long term care facilities) as determined by community and program needs. As the PHC Team evolves, and based on needs of the population, the position may be required to provide new practices such as nurse-led extended hour clinic services.   Primary Health Care services include: health assessment, health promotion and education, disease and injury prevention and assessment, treatment interventions; collaboration with, and referral to, other health care providers; participation in quality improvement, patient safety, community development, clinical practice redesign, and evaluation.
Required Qualifications

Advanced Cardiac Life Support (ACLS)
Basic Life Support (BLS)
Registered Nurse - Nurse Practitioner Masters Program
Registered or eligible for registration with the College of Registered Nurses of Saskatchewan (CRNS)
Valid Class 5 driver's license
Experience
Minimum of three (3) years experience working as a Registered Nurse, with experience as RN (NP) in a Primary Health Care setting preferred.

Ability to competently use computers and software applications as required by the position
Ability to develop and maintain good interpersonal relationships and communication with other health care professionals
