Facility: Station 20 West
Job Description: The home visiting social worker provides direct client services including ongoing assessments, developmental screening of children, curriculum delivery, client goal setting and skill building, and coordination of and support of health and social services. The home visiting social worker acts as client advocate while encouraging self motivation and direction.
Required Qualifications

Bachelor Degree in Social Work
Registered or eligible for registration with the Saskatchewan Association of Social Workers (SASW)
Knowledge, Skills and Abilities

Ability to adapt to changing conditions
Ability to assist and develop policies and procedures according to best practices
Ability to communicate with and maintain an effective working relationship with clients from diverse social, economic and cultural backgrounds
Ability to work independently and as a member of a multi-disciplinary team
Advanced communication skills
Advanced problem solving skills
Knowledge of the health region and community resources

Field Hours
Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement.  HSAS seniority within the Saskatchewan Health Authority will be considered under the Selection Criteria.

The Saskatchewan Health Authority (SHA) requires a criminal record check (CRC) from every potential SHA team member.
