Description

As part of a collaborative multidisciplinary team, utilizing an outreach approach, the goal of the Case Manager is to support clients in recovery as well as in their relationship with the OARS multidisciplinary team.  The Case manager provides addiction counselling and treatment support to the target population of those who are accessing services from the OARS (Opioid Assisted Recovery Services) clinic; at risk for HIV /Hep C, have difficulty engaging or have disengaged from care, and may be active in opioid substance use (primarily through injection drug use);  

The Wellness Counsellor/Case Manager will have sound knowledge of HIV, Hep C, and addiction recovery with methadone/suboxone as part of the recovery process, knowledge in concurrent disorders, clinical practices in addictions, case management, referrals, counselling, and assessment.  Requires ability to interpret interpersonal and intercultural dynamics and implement therapeutic strategies to ensure appropriate treatment planning.  Skills in program planning, collaboration, communication, and leadership are required. The incumbent will have developed counselling skills.

Qualification and Experience:
* A minimum of a bachelors degree in Social Work, Psychiatric Nursing, or other comparable discipline with related experience
* A minimum of two (2) years experience working with marginalized populations affected by addictions in a health-related field
* Knowledge of the Chronic Disease Management and behaviour change models, addictions, harm reduction theory, teaching/learning principles and motivational interviewing
* Demonstrated counselling theories and skills and ability to develop therapeutic relationships and support clients in meeting their goals.
* Demonstrated knowledge of community resources and programming supportive of this client population
* Demonstrated performance of appropriate documentation, record keeping and data collection functions
