Description

Flying Dust First Nation Health and Wellness Department is seeking a highly motivated and dedicated individual to provide services within the community, as the Community Health Support Worker. Their primary focus is supporting the Community Health Nurses and assessing the health promotional needs of the community.

Duties include:

Take a lead role in community development initiatives, which include forming partnerships and recognizing and utilizing resources in the community.

Uses the determinants of health and a holistic approach to health promotion.

Assist the nurses in education & awareness, coordination of baby clinics, adult wellness days.

Maintain daily and weekly records of work activities, as well as submit monthly reports on program activities.

Ensure confidentiality of all client information

Plan, develop, implement, and evaluate health promotion programs for Flying Dust First Nation targeting all age groups.

Facilitate health promotion activities for the community of Flying Dust First Nation.

Attend meetings as required, case conferences and share information within the required health teams.

Qualifications:

- Diploma or certificate in a Health Care Administration, LPN (Licensed Practical Nurse), Home Health Aide Certificate, or community development

- 3-5 years of work experience in community health promotion and education or directly related experience.

- Knowledgeable in prenatal/postnatal health and education and a thorough understanding of childhood developmental milestones.
