Job Description: The PHC RN is an integral member of the primary health care team, providing and coordinating initial, continuing and comprehensive nursing services to patients in the PHC sites.
The PHC RN promotes quality health care outcomes working within established standards for case management practice.
The PHC RN initiates and/or participates in community development initiatives and participates in the evaluation of PHC initiatives within the site.
The PHC RN provides clinical education and support to the medical office assistants.
Required Qualifications
Bachelor Degree in Nursing
Registered or eligible for registration with the College of Registered Nurses of Saskatchewan (CRNS)
Valid Class 5 driver's license
Knowledge, Skills and Abilities
Ability to adapt to changing conditions
Ability to competently use computers and software applications as required by the position
Ability to work independently and as a member of a multi-disciplinary team
Assessment skills
Communication skills
Interpersonal skills
2 years nursing experience in a community based health care setting preferred. Knowledge of chronic disease management. Experience in case management an asset. Level C, Basic Cardiac Life Support. The Saskatchewan Health Authority (SHA) requires a criminal record check (CRC) from every potential SHA team member. The CRC must be dated within the past six (6) months and be satisfactory to the SHA. A vulnerable sector check (VSC) is required for individuals providing patient/resident/client care to vulnerable persons.
