Description

Sanctum is a non-profit organization that provides transition, respite, and palliative care to HIV positive individuals.

The home support worker will provide care and support to residents with or at high risk for HIV/AIDS, who may also be affected by mental illness, addictions, poverty, homeless or other psycho-social factors. The home support worker will embody a patient centered harm reduction care model aimed at creating independence and autonomy while fostering an environment of acceptance and understanding for residents.

This position will require evening, night, weekend and shift work.  

Responsibilities
Personal care including: assisting with grooming, bathing, personal hygiene, toileting, wound care, nutrition and all other activities associated with daily living.
Assist with and support clients with medication adherence.
Work with patients on goals and care plans aimed at recovery and management of their illness(s)  
Build rapport with residents and provide therapeutic counselling for multiple issues affecting their ability to cope and manage in the community.
Advocate on behalf of clients for additional service and resources, as required.
Establish and maintain current, accurate, confidential files for each resident.
Assist with transition plans into the community set by the client and care team.
Follow medical instructions carefully and consistently.
Assist with meal preparation and planning.
Household duties such as cleaning and maintenance
Manage any unanticipated events or unstable situations.
Care for home therapy dogs
