Job Description: The Assessor Coordinator reports directly to the Home Care Manager.  The Assessor Coordinator is responsible for assessment/reassessment services with ongoing client care planning and management.  The Assessor Coordinator is accountable for establishing and maintaining positive liaison between the Health Centre and Home Based Services.  The Assessor Coordinator is accountable for screening, referring, reviewing, planning and facilitating safe care for individuals.
Required Qualifications

Bachelor Degree in Occupational Therapy, Physical Therapy, Social Work, Nursing or Psychology or a related field
Bachelor Degree/Diploma in Nursing or Bachelor Degree/Diploma in Psychiatric Nursing or Bachelor Degree in Social Work or Licensed Practical Nurse with 4 years experience
Registered or eligible for registration with the appropriate licensing body

Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement. HSAS seniority within the Saskatchewan Health Authority will be considered under the Selection Criteria.

The Saskatchewan Health Authority (SHA) requires a criminal record check (CRC) from every potential SHA team member. The CRC must be dated within the past six (6) months and be satisfactory to the SHA.
