You will be joining a dedicated team of community focused specialists with a desire to build a stronger community with our local seniors in Moose Jaw and rural area. We aim to have Moose Jaw a connected community that supports Seniors Citizens to remain in their own homes.
This is a new start up position that will assist seniors in independent living and wellbeing.
RESPONSIBILITIES Dual Role of community outreach and Office administration
Do intake assessments and follow up on them
Support seniors in finding and accessing social, economic, recreational and educational services and resources in the community
Scheduling, supervise staff and volunteers
Leads/Collaborates on Community Engagement Activities
Project tasks involving project planning, reporting and evaluation
Works collaboratively with others to identify, organize and facilitate social activities, support groups, and educational workshops that foster wellbeing, a sense of community and help overcome social isolation among seniors.
Identifies unmet needs and collaborates with system partners to develop and/or deliver educational materials to empower individuals with knowledge and skills.
Links seniors to the appropriate resource (ie: Care Coordinator, Community Relations Worker, Home Care, etc.) to facilitate referrals and access to community programs, social services, and health care options.
