Through funding made possible by Saskatchewan Seniors Mechanism, Age Friendly Moose Jaw and The Moose Jaw & District Seniors Association have entered into an agreement to be a pilot project for Social Prescribing for older adults. This 3.5 year project will include the hiring of a half-time contract position (20 hours/week) to implement social prescribing services for older adults in the community.
The Community Connector seeks to empower clients to take control of their health and well-being through a holistic approach. The focus is connecting them with community groups and services for practical and emotional support. The position will assist older adults to find, access, and/or create support services with local providers. A comprehensive job description will be provided to candidates selected for interviews.
Candidates must have a valid Class 5 drivers license, a vehicle of reasonable quality in good repair, and the ability to travel on a regular basis. Any successful candidate will be required to provide criminal record and vulnerable person checks dated within the last 6 months.
