Facility: INVERMAY HEALTH CENTRE
Job Description: Reporting to the designated Health Service Manager (HSM)/Health Service Administrator (HSA), the Clinical Care Coordinator (CCC) is responsible for ensuring that the highest standard of care and services are provided to residents/clients.

As an experienced practitioner, the CCC acts in a leadership role to provide specialized clinical expertise, coordination of resident care, and support and teaching to the resident care team (including residents, resident's family members, physicians, pharmacists, nursing and all other facility staff) within a collaborative environment. The routine procedures are generally predetermined but critical thinking and independent judgement is an integral part of the position.

Required Qualifications

Bachelor of Nursing or graduate of an accredited school of nursing.
Current BCLS certification
Registered or eligible for registration with the College of Registered Nurses of Saskatchewan (CRNS) or Registered Psychiatric Nurses Association of Saskatchewan (RPNAS)
Knowledge, Skills and Abilities

Ability to travel both within the region and provincially
