Facility: COMMUNITY/PUBLIC HEALTH EAST
Job Description: Under the direction of the Manager of Rehab and Residential Services the Assessor Coordinator( MHT) will be responsible to assist in the successful transition of acute psychiatric patients from hospital into community or alternatively to prevent admission/readmission by providing short term acute mental health in home/ community services. Service provision includes: screening, developing care plans, coordination with other service providers and direct mental health homecare support.
Required Qualifications

Recognized degree/diploma in nursing or social work degree
Registered with the appropriate professional licensing organization
Experience
Under the direction of the Manager of Rehabilitation Services , the Assessor Coordinator for Mental Health Transitions will be responsible to assist in the successful transition of acute psychiatric patients from hospital into community or alternatively to prevent admission/readmission by providing short term acute mental health in home/community services.  Service provision includes:  screening, developing care plans, coordination with other service providers and direct mental health support.
Knowledge, Skills and Abilities

Able to work independently and in a self-motivated manner, ability to respond under pressure
Basic computer skills
Demonstrated above average clinical assessment skills, case planning, consultation and supervision  
Demonstrated leadership, communication, decision making, documentation and organizational skills
Excellent verbal and written communication skills
Good public relation skills and ability to establish and maintain effective work relationships
Knowledge and practical application
