Job Description: Under the leadership and direction of the Director - Five Hills Access Centre, the incumbent is responsible to act as the primary liaison between physicians, facility nursing unit managers, facility health care providers, and Continuing Care services (Home Care, Transition Programs - respite, waiting placement, convalescence, palliative care, long term care, Geriatric Assessment and Rehabilitation Unit, Adult Day Program) The Transition Coordinator facilitates the transition/ discharge planning process from the acute care facility back to the community or Long Term Care in accordance with Five Hills Health Region (FHHR) policies, procedures, and guidelines.
Required Qualifications

Bachelor Degree/Diploma in Nursing or Bachelor Degree/Diploma in Psychiatric Nursing or Bachelor Degree in Social Work or Licensed Practical Nurse with 4 years experience
Registered or eligible for registration with the appropriate licensing body
Knowledge, Skills and Abilities

Ability to work with people in a health care environment with professionalism and respect
Conflict resolution skills to effectively handle difficult issues
Effective oral and written communication skills
Excellent assessment, diagnostic and intervention skills
Good interpersonal skills utilizing sensitivity and courtesy when dealing with patients, staff and physicians
Knowledge of the health region and community resources
Leadership skills
Positive approach and good listening skills to deal with challenging situations with empathy and sincerity
Time management skills
