The Family Preservation Worker focuses on providing supports and services to vulnerable and at-risk families, through flexible and intensive home visitation, referrals and collaboration with other community services, so to assist the family to develop the skills necessary to keep children safe and to keep the family intact when it is possible to do so. This position will collaborate with members of an interdisciplinary team to plan, implement, coordinate, and evaluate client care and support in consultation with individuals and their families.

Effective interpersonal and communication skills

Assist with or provide transportation if needed
Refer to and coordinate with other agencies as required

Observe program and organization policy and procedure and OH&S standards
Qualifications, Experiences and Requirements:
Post-secondary education in a human service related field or an equivalent combination of work experience and education
Valid Drivers License and a safe, reliable vehicle
Minimum two-years experience working with children and families
Experience in case management, documentation and file maintenance
Recent (6 months or less), clear Criminal Record Check and Vulnerable Sector Check
