Description

We are looking for a full time Registered Nurse to join the teams in our collaborating Clinics. In this role, you will work together with physicians and other healthcare professionals to provide top-quality care, health education, and support to patients.

What doing a great job will look like? (Key responsibilities)
-Assist in the coordination and delivery of Chronic Disease Management (CDM) for patients with Diabetes, Heart Failure, Chronic Obstructive pulmonary disease (COPD) and Coronary artery disease.
-Conduct patient reviews, including medical histories, vital signs and appropriate lab tests.
-Assist in the delivery and completion of individualized assessment tools such as the Rourke Baby Records, MoCA (Montreal Cognitive Assessment form for Cognitive Impairment and Dementia).
-Administer injections and vaccinations to adults, and other treatment as directed.
-Perform procedures as removal of sutures and staples, ear syringing and simple wound dressing.
-Prepare examination rooms and assist Physicians during exams and procedures.
-Document patient information accurately in the EMR (electronic medical records)
-Provide health education to patients.
-Assist Physicians with EMR tasks, forms etc.
