Description

Key Duties and Responsibilities:
    Assessment and Intake:
    Conduct comprehensive psychosocial assessments of clients to identify their needs, strengths, and challenges.
    Gather relevant information from clients, families, and other sources to inform treatment planning.
    Determine eligibility for services and programs offered by Clear Health Solutions Ltd. and Sheltering Wings.
    Complete all necessary documentation in a timely and accurate manner, adhering to privacy legislation and professional standards.
    Treatment Planning and Intervention:
    Develop individualized treatment plans in collaboration with clients, outlining goals, objectives, and intervention strategies.
    Provide individual and group counseling using evidence-based practices to address a variety of mental health, social, and emotional issues.
    Implement crisis intervention strategies as needed to ensure client safety and well-being.
    Monitor client progress and adjust treatment plans as necessary.
    Case Management and Advocacy:
    Connect clients with appropriate community resources, such as housing, financial assistance, and support groups.
    Advocate on behalf of clients to address systemic barriers and ensure access to necessary services.
    Collaborate with other healthcare professionals, schools, and social service agencies to provide coordinated care.
    Maintain accurate and up-to-date client records.
    Professional Development and Ethics:
    Adhere to the SASW Code of Ethics and maintain professional boundaries.
    Participate in ongoing professional development activities to enhance knowledge and skills.
    Engage in supervision and consultation with other professionals to ensure best practices.
