The Housing Support Worker provides a wide array of supports to individuals who are housing insecure. Provide support services to individuals who access Square One Community's housing program - WEEKEND AND EVENINGS.
Monitor clients individualized Recovery plan and assist in the areas of daily living including financial, vocational, social support, cultural, wellness, mental health and addictions, and/or recreational supports
Establish and maintain effective working relationships in a multi-disciplinary team environment by providing comprehensive supports to all clients participating in the program.
Provide appropriate, professional responses to clients disclosing thoughts of self-harm or suicide, hallucinations, delusions, or other symptoms. Consult with the Intensive Case Manager as needed.
Develop and maintain trusting, healthy, therapeutic relationships with clients
Attend case conferences, appointments, and other client meetings as needed.
Facilitate support groups aimed at improving client quality of life.
Consult with the Intensive Case Managers to facilitate clients referral to emergency services such as the PACT team, the ER and rapid counselling during times of crisis, acute mental health challenges, and suicidal ideation.
Provide physical assistance as needed to clients in the areas of grocery shopping and apartment maintenance.
Organize and participate in moving of clients as required.
Work with landlords or other housing providers to enhance landlord-tenant relationships, provide advocacy, and ensure sustainability of housing.
Assist clients with their medication management.
Maintain complete, accurate, and comprehensive case notes.
Establish and maintain effective collaborative working relationships with other partner agencies.
Attend and actively participate in meetings as required.
Attend mandatory training requirements.
Administration tasks as required.
Other duties may be assigned.
