THE POSITION
The Client Navigator, under the direction of the STC Health Centre Manger, will be primarily responsible for assessing client needs and support systems and assisting clients to reach their identified health goals, acting as a resource to navigate social service and healthcare systems and ensuring that they have the supports they need to optimize their health and wellbeing.  The Client Navigator will work to improve access to internal and external supportive services for clients, improve health outcomes by removing barriers to access, promote early intervention that will mitigate the need for higher intensity support later on and reduce health disparities by ensuring access to services that are appropriate and available where and when clients need them.
